You will need to create an access key with the Access Key Request Form before filling out most of the forms. Once you're given an access key, you must paste it at the top of each form you fill out. If you forget what it is, you can always use the Access Key Retrieval Form to retrieve it.

Why do I need an Access Key?
Some individuals filling out form(s) don't have accounts with Franciscan University, so the access key serves as a way to group the forms that they've completed together.
