If you suspect a child is being abused or neglected, you are legally obligated to make a prompt (within 24 hours) report by calling 1-855-OH-CHILD (1-855-642-4453), to be connected to a local child welfare or law enforcement agency. You also can report abuse or neglect by directly contacting the law enforcement agency or child welfare office in the county where the incident occurred or where the child and his or her parent, guardian or custodian live. It is also extremely important to notify the University’s Youth Protection Officer at 740-283-6238 immediately in these situations, regardless of your campus/activity/program location.
In addition to these reporting obligations, this form is to be completed by the individual who witnesses, or has substantive information of suspected child abuse, neglect, and/or sexual abuse of a minor. Make sure that the information provided below is factual and detailed as possible. Maintain a copy for your records.
